Mounjaro savings card in 2026: who qualifies and what you pay
There is no Mounjaro coupon code; the discount that exists is Lilly's Mounjaro Savings Card, which can bring the price to as little as $25 for people with commercial insurance that covers the drug. People on Medicare, Medicaid or other government coverage cannot use the card, and the list price is about $1,112 for a 28-day supply.

Is there a Mounjaro coupon code?
No. Mounjaro (tirzepatide) is a prescription medicine from Eli Lilly, and prescription drugs are not sold with promo codes the way retail products are. Any site offering a Mounjaro code to type at checkout is not offering a real discount from Lilly.
What Lilly does offer is a manufacturer savings card that a pharmacy applies to an eligible prescription, plus a self-pay option through LillyDirect. Both are described on Lilly's Mounjaro savings and resources page. PickedBox does not run a Mounjaro coupon page for this reason.
What Mounjaro costs before insurance
Lilly's Mounjaro FAQ gives a list price of $1,112.16 for a 28-day supply, which is four single-dose pens. Lilly's pricing information page explains that list price is what Lilly charges wholesalers, not what most patients pay at the counter. A January 2025 state disclosure sheet listed $1,079.77 per carton, so the figure has moved up since then.
The list price is the same for every dose strength. What a patient actually pays depends on the insurance plan, the pharmacy and whether a savings program applies.
How the Mounjaro savings card works
The card has two levels for people with commercial insurance, such as a plan through an employer or the marketplace. These figures come from Lilly's savings pages at the time of checking (October 2026).
| Situation | Pay as little as | Savings limits |
|---|---|---|
| Commercial plan covers Mounjaro | $25 for a 1-, 2- or 3-month prescription | Up to $150 per 1-month fill, $300 per 2-month fill, $450 per 3-month fill, and $1,950 per calendar year |
| Commercial plan does not cover Mounjaro | $499 for a 1-month fill (28 days, up to 4 pens) | Lilly lists a monthly maximum of $647 and an annual maximum of $8,411 |
Once the annual savings limit is reached, the card stops paying and the regular copay applies. Lilly's terms allow up to 13 fills per calendar year. Lilly's savings page explains how to get the card, and the pharmacy applies it when filling the prescription.

Who qualifies for the savings card and who does not
Lilly's special terms set out the main conditions:
- Commercial insurance: the card is for people with commercial drug coverage. Someone without any insurance is pointed to the self-pay option instead.
- No government coverage: the card cannot be used by anyone enrolled in Medicaid, Medicare, Medicare Part D, Medicare Advantage, Medigap, DoD, VA, TRICARE or a state prescription assistance program.
- A valid prescription: the card applies to Mounjaro, which the FDA approved for adults with type 2 diabetes. Lilly's weight management medicine, Zepbound, has its own savings program.
- Expiration: the current card expires and savings end on Dec 31, 2026.
- Terms can change: Lilly states that it may end or amend the program at any time without notice.
The card is not insurance and cannot be transferred to another person.
Mounjaro with Medicare or Medicaid
People with Medicare or Medicaid cannot use the savings card, but that does not mean they pay list price. Many Medicare Part D plans cover Mounjaro for type 2 diabetes. Lilly's FAQ states that about 8 out of 10 Part D patients pay between $0 and $50 for a 28-day supply.
For people whose Part D costs are high early in the year, the Medicare Prescription Payment Plan spreads out-of-pocket drug costs into monthly payments across the plan year. It is free to join through the Part D plan and does not lower the total amount owed.
In November 2025, the federal government announced a pricing agreement with Lilly and Novo Nordisk that included a $245 monthly Medicare price for these medicines, CBS News reported. How that shows up for an individual depends on the plan, so the Part D plan's own drug list and copay are the figures to check.
Paying cash through LillyDirect
For people with no insurance, or a plan that will not cover Mounjaro, Lilly sells the pens through its LillyDirect self-pay program. The LillyDirect page lists every dose from 2.5 mg to 15 mg starting at $499 a month, with terms that apply. A prescription is still required, and the order ships from a partner pharmacy.
That $499 figure matches the savings card price for commercially insured people without coverage, so a person in that group should compare both and pick whichever has the simpler terms for their plan. Pharmacy discount cards also list Mounjaro prices, but those prices vary by pharmacy and should be compared against the $499 figure before filling.

Questions to ask the prescriber, pharmacy and insurer
- Is Mounjaro on the plan's drug list, and is prior authorization required?
- What is the copay for a 1-month and a 3-month supply?
- Does the pharmacy accept the Lilly savings card, and will it run the card alongside the insurance?
- If coverage is denied, is there an appeal route or a covered alternative?
- For people with low income and no coverage, does the Lilly Cares Foundation include Mounjaro on its current list? Sources disagreed on this at the time of checking.
This guide covers cost and eligibility only, not medical advice. Whether Mounjaro is the right treatment, and at what dose, is a question for the prescriber.
How we checked this
In October 2026, PickedBox reviewed Lilly's Mounjaro savings and resources page and special terms, the Mounjaro FAQ, Lilly's pricing information page and state disclosure sheet, the LillyDirect Mounjaro page, the Medicare Prescription Payment Plan page and news coverage of the November 2025 pricing agreement. Lilly's sites returned access errors to automated requests, so their figures were taken from the official pages as shown in search results and should be confirmed on the live pages. Nothing was purchased and no medical claims are made.
